What is Social Prescribing?

Our expert Social Prescribers work alongside you to identify your priorities and what matters to you.
We help you to access the support and connections in our community that can help you to manage your health & wellbeing.

Around 80% of health outcomes are caused by something other than medical care – for example housing, money, social
support. We are here to help you to address your challenges, connect you to the right help, and support you to feel better.

What we do:

Our job is to understand what matters to you, and stand by your side to help you to access the support you need.

What we don’t do:

We are not specialist advisors in money, housing or medical matters – but we can link you to professionals and organisations or community groups that can help.

Who is Social Prescribing for?

Social Prescribing can help people

  • who live with one or more long-term conditions
  • who need support with their mental health
  • who are lonely or isolated
  • who have non-medical needs that affect their health & wellbeing

Who is Social Prescribing not for?

  • People whose primary concerns are severe and clinical
  • People whose urgent needs (e.g. mental health) have not yet been referred for assessment to a suitable service
  • People whose behaviour is likely to be volatile or harmful to those around them
  • People who are unwilling to give something new a try

Your Social Prescriber (or ‘Link Worker’) is attached to your doctor’s surgery, and will give you an email/phone number so you can contact them. They will support you for a period of weeks or months, once any urgent needs are being addressed by your clinical
team.

Social Prescribing at WellBN

Social prescribing at WellBN is delivered by HERA and Robin Hood Health. We have been working together for almost a decade now in providing hollistic care for our patients.
