At WellBN, caring for you means more than treating illness. It means listening, understanding what matters to you, and supporting you and your loved ones through every stage of life – including the most difficult and emotional times.

We are proud to be recognised as a Daffodil Practice, which means we meet nationally recognised standards for providing high-quality, compassionate care for people living with serious illness, those approaching the end of life, and those who are bereaved.

But what does this actually mean for you?

What Is a Daffodil Practice?

The Daffodil Standards are a set of best-practice principles developed to help GP practices deliver consistently kind, coordinated, and person-centred care for people with serious or life-limiting conditions.

Being a Daffodil Practice means WellBN has carefully reviewed how we support patients and families — not just medically, but emotionally, socially, and practically – and made real improvements to ensure you feel supported, informed, and respected.

Why This Matters

We understand that serious illness, frailty, or the end of life can be overwhelming. Patients often tell us they worry about:

  • Being a burden
  • Losing control
  • Not being listened to
  • Their family not being supported
  • Not knowing what will happen next

As a Daffodil Practice, we commit to addressing these fears with honesty, kindness, and clear communication. Our aim is to help you feel safe, heard, and cared for – not rushed or overlooked.

What This Means for You as a Patient

1. You Are Treated as a Person, Not a Condition

We take time to understand what matters to you, not just what is the matter with you. This includes:

  • Your values and wishes
  • What quality of life means to you
  • Your cultural, spiritual, or personal beliefs
  • Who is important in your life

We involve you in decisions about your care and respect your choices.

2. Better Communication and Planning

We don’t wait for a crisis to talk about the future. When appropriate, we:

  • Have gentle, honest conversations about what lies ahead
  • Discuss your wishes for future care
  • Help you create personalised care plans
  • Make sure your preferences are recorded and shared with the right services

This helps avoid unnecessary hospital visits and ensures your care reflects what you want.

3. Joined-Up Care

We work closely with:

  • District nurses
  • Community teams
  • Hospices
  • Social services
  • Pharmacies
  • Specialist teams

This means you don’t have to repeat your story again and again – and your care is better coordinated.

4. Support for Families and Carers

Illness affects more than just one person. At WellBN, we make sure that carers and family members:

  • Are included in conversations (with your consent)
  • Know who to contact for help
  • Receive emotional and practical support
  • Are supported before and after a bereavement

5. Kindness Is Not Optional Here

Every member of our team – from reception to clinicians – understands the importance of compassion, patience, and dignity. We know that small acts of kindness make a big difference.

What We Have Done to Achieve This

Becoming a Daffodil Practice is not a tick-box exercise. It involved real reflection and change.

At WellBN, we have:

✔ Improved How We Identify and Support Vulnerable Patients

We proactively identify patients who may need extra support due to frailty, serious illness, or complex needs — so help is offered early, not too late.

✔ Introduced Regular Multidisciplinary Team Meetings

We discuss complex cases as a team, ensuring everyone involved in your care is aligned and informed.

✔ Strengthened Our Care Planning Process

We ensure care plans are meaningful, personalised, and actually used – not just stored away.

✔ Improved Access to Support

We make it clear who to contact, what to do in a crisis, and how to get help quickly.

✔ Trained Our Team

Our staff have received training in:

  • Compassionate communication
  • End-of-life conversations
  • Supporting bereaved patients
  • Recognising emotional distress

✔ Listened to Feedback

We have learned from patient and family feedback to shape how we deliver care.
