Usually, the practice will contact you when you are due your annual review for your long-term or chronic health condition. Sometimes conditions will be reviewed more often depending on severity or other reasons.

However, if you wish to have a review for any of the following conditions then please complete a questionnaire online at your SystmOnline account (https://systmonline.tpp-uk.com/) or Airmid app which can be found in the surveys section.

The following conditions have associated review questionnaires:

• Asthma
• COPD (Chronic Obstructive Pulmonary Disease
• Cardiovascular Disease including Hypertension
• Heart Failure
• Diabetes
• Epilepsy
• Depression / Anxiety
• Oral Contraception
• HRT (Hormone Replacement Therapy)

You can complete one of these questionnaires or complete the following box with extra details regarding the reason for your review. You will be contacted within 5 working days with a response.

Note: if you are experiencing significantly worsening symptoms, please complete the 'new health problem' option which will be reviewed more quickly.
